Substance Abuse Services

We provide face-to-face assessments and evaluations with SAPs (Substance Abuse Professionals) for employees who have violated a Department of Transportation drug and alcohol regulation. 

The DOT substance abuse services we provide are geared toward timely and efficient services, problem-solving, creating plans, moving into action, and returning to safety-sensitive duties.

The SAP Evaluation

~1.5 hours

The employee will be sent intake paperwork to complete ahead of the appointment. Our SAP counselor will conduct a face-to-face assessment and clinical evaluation, and recommend a course of education and/or treatment. They will give referrals to complete a urine test off-site, and develop a follow up testing plan, providing both employee and employer with guidelines for continuing care.

Follow-up Meeting

~30 min

During follow-up meetings, the SAP couselor will review the outcomes of the treatment recommendations. If complete, they will submit a written report to the current or previous employer, indicating the employee has met all the SAP recommendations and is eligible to return to duty. Subsequent meetings may be scheduled based on compliance and completion of recommendations.

DOT / SAP Evaluation +Follow-up Meeting: $650

Full payment is due at the time of scheduling. Evaluations are private pay. Subsequent Follow-up Meetings $100 / 30min

  • A SAP is a qualified Substance Abuse Professional that performs mandated evaluations for employees who have violated the Department of Transportation (DOT) drug and alcohol program regulation. A SAP has completed the training and testing required by DOT.

    While a diagnosis can sometimes help make sense of symptom patterns—and may be required for insurance reimbursement—it’s not a prerequisite to receive care. At NEO Mind Body Collab, we see diagnoses as just one piece of the puzzle. We’re far more interested in understanding your unique story, how your experiences have shaped you, and how we can support healing in a way that feels safe, empowering, and personalized.

  • A SAP conducts a face to face assessment and clinical evaluation of the employee. The SAP will then recommend a course of education and/or treatment. The SAP serves as a referral source to assist the employee

    with recommendations, case-management and intervention monitoring while you are in the program. The SAP will conduct face to face follow up sessions with the employee to determine compliance with recommendations. The SAP also develops a follow up testing plan for the employee and provides the employee and employer with recommendations for continuing care.

  • The length of time necessary depends upon the specific intervention required. The objective is to return the employee to safety sensitive duty at the earliest date following successful completion of their assigned intervention.

  • A DOT SAP Evaluation and Follow-up Meeting is $650. Subsequent Follow-up Meetings are $100 per 30-min. Full payment is due at the time of scheduling. Evaluations are private pay.
